Our Web site is only set up to accept online payments for new member applications. If you choose to pay by check, please following the guidelines below.

• Print out the MN Chapter NAPO membership application as a PDF file. In order to open the file, you must have Adobe Acrobat Reader. This free program can be downloaded from here.
• If you are unable to download and print the PDF file or would like to receive membership information by mail, please complete our Online Request Form.
• Upon completion of this application, please send payment and application to the following address:

• Upon receipt of application, our Membership Chair will contact you to let you know the state of your application.
• Applications are reviewed and approved at monthly meetings by the MN Chapter NAPO board members.
• Applications cannot be approved until NAPO membership has been processed through the national association.
